Form Embed
The Form Embed Block allows site administrators to embed custom, university-approved forms directly into a webpage. These forms can include tailored fields and data-collection requirements specific to the needs of a department or initiative.
Because embedded forms interact with university data systems and may collect sensitive or personally identifiable information (PII), all Form Embed Blocks must be reviewed and approved by the Web Services team before they are published.
When to Use This Component
- You need to collect information from users beyond what standard form tools provide
- Data collected must be stored or processed through approved university systems.
- Do not use this component if a simple link to an existing form (Google Doc) meets the need or if the form has not yet been cleared by Web Services.
Do’s and Don’ts
DO
Consult Web Services early before creating or requesting a custom form, and be sure to provide clear requirements describing what information you need, how it will be used, and where the data must go. Ensure the form is fully accessible, with meaningful labels, logical tab order, and screen-reader compatibility, as this will be verified during approval. Test the form thoroughly before requesting publication, and use the Form Embed Block only for official university business, not for casual surveys or one-off questionnaires.
DON’T
Don’t publish a Form Embed Block without explicit Web Services approval, and don’t collect sensitive information such as Social Security numbers, medical data, or financial records unless it is approved and required for a sanctioned university workflow. Avoid duplicating existing university forms or systems by first checking whether a standard tool already meets the need. Do not assume the form will display correctly on all devices without testing to verify responsive design, and never embed unauthorized third-party forms that fail to meet university privacy and security requirements.
How To Use This Component
Submit a trouble ticket to Web Services and be sure to include the following information:
- Name of the college and department
- URL where the form should appear
- Purpose of the form — what you need it to accomplish
- Details about the data being collected
- A complete list of required fields
- Where the collected data should be sent or stored
- A clear, descriptive title for the form
If your request is approved:
- Add a Form Embed block to the page.
- In the block settings, locate the Form Type dropdown menu.
- Select your form from the list, it will appear under the title you provided.
